As a courtesy to our patients, we will do our best to bill your insurance for you. Please come to each appointment with updated insurance information including any household changes that may have an effect on your insurance plan.

NOTE: All minors must be accompanied by a parent or legal guardian for the entirety of their visit. This parent/guardian will be responsible for full payment when services are rendered. Full payment is ultimately the responsibility of the patient/guardian regardless of insurance coverage.
